To enroll in our medical plan, please print the following form, fill it out completely, and send it to Webfoot Staffing Solutions, P.O. Box 31351, Seattle, Washington 98103. You can also fax the completed form to us at 206-524-1466. Do not forget to sign and date it where indicated (boxed area near the bottom of the first page), and if you have prior medical insurance please fill out the second page.

Vacation & Holidays
Most of our contract offers contain a vacation and holiday package. Your Webfoot recruiter will tell you if the contract you are being offered includes this.

Short Term Disability
We offer Short Term Disability insurance to all of our employees at no cost because we understand that life happens!
